Dress code 2010/2011

Tiny Steps

Solid colored leotard with or without skirt, pink ballet slippers, and thin single fold white socks. Hair must be pulled back from face in pony tail, pig tails or a bun.

Creative Movement levels 1 and 2 (pink)

Cap-sleeved dress by MotionWear (style: 4354 color: light pink), pink ballet slippers, and thin single fold white socks (NO TIGHTS). Hair must be pulled back from face in pony tail, pig tails or a bun.

Pre-Ballet (white)

Cap-sleeved dress by MotionWear (style 4354 color: white), pink tights, and pink leather ballet slippers. Hair must be in a ballet bun. Short hair must be pulled back from face.

Primary (butter)

Tank leotard by MotionWear (style 2100 color: butter), matching skirt (style 1011), pink tights, and pink leather ballet slippers. Hair must be in a ballet bun. Short hair must be pulled back from face.

Level 1 (candy pink)

Tank leotard by MotionWear (style 2200 color: candy pink), matching skirt (style 1011), pink convertible foot tights, and pink leather ballet slippers. Hair must be in a ballet bun. Short hair must be pulled back from face.

Level 2 (light blue)

Tank leotard by MotionWear (style 2200 color: light blue), matching wrap skirt (style 1022), pink convertible foot tights, and pink leather ballet slippers. Hair must be in a ballet bun. Short hair must be pulled back from face.

Level 3A and 3B (periwinkle)

Tank leotard by MotionWear (style 2200 color: periwinkle), matching wrap skirt (style 1022), pink convertible foot tights, and pink leather ballet slippers. Hair must be in a ballet bun. Short hair must be pulled back from face.

Level 4 (hunter)**

Hunter silkskyn camisole leotard by MotionWear (style 2518K color 479), pink convertible foot tights, and pink leather ballet slippers. Skirts will be available at TBS only. Hair must be in a ballet bun. Short hair must be pulled back from face. Skirts may be worn for pointe classes.

Level 5 (maroon)**

Maroon silkskyn camisole leotard by MotionWear (style 2518K color 490), pink convertible foot tights, and pink leather ballet slippers. Skirts will be available at TBS only. Hair must be in a ballet bun. Short hair must be pulled back from face. Skirts may be worn for pointe classes.

Levels 6 and 7 (black)**

Black silkskyn camisole leotard by Motionwear (style 2518K), pink convertible foot tights, and pink leather ballet slippers. Skirts will be available at TBS only. Hair must be in a ballet bun. Short hair must be pulled back from face. Dancers may wear any color/style leotard on Saturdays. Skirts of any color may be worn for pointe classes.

Boys attire for all levels:

Plain white t-shirt tucked inside black exercise shorts with white ankle socks and black ballet shoes.

Or, plain white t-shirt tucked inside boys black tights with black ballet slippers.

Modern/Jazz classes:

Black footless tights worn over ballet uniform.

Ballet slippers or jazz shoes for Jazz classes and bare feet for Modern classes

Open Teen/Adult Ballet, Ballet Basics and Pilates: Standard Ballet clothes or comfortable close fitted workout clothes (ie yoga pants, tank top for ladies and shorts, plain t-shirt for men) with ballet shoes, bare feet or socks.

All attire for girls in level 3B and below may be purchased from The Ballet Space. Pre-Order Uniforms before July15th for a discount! Ballet slippers may also be purchased at The Ballet Space for children’s sizes 7 - 1.5 only. Attire/shoes for all dancers can also be purchased at a ballet supply store. Please see list on the reverse side of this paper for a store near you. Do not purchase shoes from Payless or other such stores as they are not a proper fit, material or color. **Ballet 4-7 may order their uniform from The Ballet Space prior to July 15^th to ensure delivery prior to the beginning of classes.

Uniform: If you would like to pre-order your dancer(s) uniform for a 10% discount from The Ballet Space, please mail in the Uniform Order Form with payment (may be on same check as deposit). You will receive an e-mail

letting you know when you can come to pick up uniforms at The Ballet Space in August/Early September. Pre-Order Discount ends July 15^th!

I hereby certify that my child is physically and emotionally capable in participating in this dance program. I hereby release, absolve, indemnify and hold harmless The Ballet Space, and any employee or volunteer, of any a ccident or injury that may occur as a result of this extra-curricular activity.

Payment/refund policy: I understand that tuition must be paid by determined due dates in order for my child to participate in class. All deposits are non-refundable. A partial refund of tuition may be given if you are not satisfied for any reason, but not after your child has completed two classes. I understand that a $10.00 late fee will be added each month to my account if payment is not received by the due date. We do not automatically charge credit cards if that was your first method of payment. A $25.00 fee will be charged on all returned checks. There are no refunds or credits for missed classes.

Payment schedule

First Steps Program:

Fall Session (12 weeks 9/8–12/5) Payment is due by Monday, September 1^st (minus deposit)

Winter Session (10 weeks 12/7–3/6) Payment is due by Monday, December 1^st

Spring Session (12 weeks 3/8–6/7) Payment is due by Monday, March 1^st

Pre-Ballet through Ballet 3B:

50% of tuition (minus your deposit if already paid) is due by September 1^st

50% of remaining tuition is due by February 1^st

Levels 4 through 7: Payment option #1

Payment policies:

~ Please keep this schedule for your records. We do not send out bills, unless requested, so that we may save on paper and postage fees. We

also do not automatically charge your credit card if that is how you made your first payment. If your payment has reached being one month late, you will receive a bill with a $10.00 late fee each month until paid.

~ Payments plans may be worked out on an individual basis for parents of pre-ballet through level 3 students if needed. Payment plans must be arranged by September 1^st.

~ There is a $25.00 fee for all returned checks.

~ All deposits are NON-REFUNDABLE.

~ There are no tuition reimbursements or discounts for absences.

~ A partial refund of tuition may be given if you are not satisfied for any reason, but not after your child has completed 2 classes.

~ All accounts must be paid in full, including any late fees, by May 1st in order for dancer(s) to participate in Spring Demonstration.
